PRESCRIPTION REFILLS:​​

A $15.00 fee applies to any prescription refill requests made outside of regularly scheduled appointments.

(In this case, refills may or may not be provided at the discretion of provider)

CANCELLATION POLICY:


A minimum of 24 hours notice is required for ALL appointment cancellations. A $65.00 fee will be assessed for patients providing LESS than 24 hours notice.


If more than 2 scheduled appointments are missed, you will be subject to termination from the practice

PAYMENTS/BALANCES:

Copays and deductible payments are due at the time of your visit.

Sometimes unforeseen charges remain after insurance processes claims, it is YOUR responsibility to clear up balances in a timely manner. Our new payment system allows for Credit/HSA cards to be run easily without the card present, and also to set up budget friendly payment options. Please contact the office to discuss payment options.

(Unpaid balances will result in the inability to schedule further appointments until all charges are paid in full.)
